Have you survived a gunshot injury? Tell us about your experience.
There are more than 80,000 non-fatal shootings each year in the United States, but little attention is paid to this public health crisis. By taking this survey, you will help journalists, doctors, policy-makers, and other survivors understand what it is like to live through and recover from a gunshot injury.
The survey will take about 8 minutes to complete. Thank you for sharing your experience with us.
Please be advised: This survey includes questions about the circumstances and aftereffects of the shooting in which you were injured. If you deal with post-traumatic stress, be aware that some questions may act as triggers.
A note about privacy: The Trace is gathering this information for the purposes of our reporting. We take your privacy seriously and your responses will be confidential. In the survey, we ask if one of our reporters may follow up with you. Your identifying information will not be shared without your permission. If we would like to identify you by name in an article, we will contact you in advance.
